Spinal Dysraphism and Congenital Abnormalities

Spinal Dysraphisms… the “-celes”

  • Lipocele

  • Myelocele

  • Myelomeningocele

  • Lipomeningocele

  • We need to address the neural placode

    • This is basically the neural tissue that is fucked up for whatever reason

    • If it remains in the spinal canal then there is no myelocele

    • If the tissue protrudes through the defect then there is a myelocele

  • s

Lipomyelocele

  • Basically within the spine communicates with the sub-Q tissues

  • If there is associated protrusion of neural tissue (neural placode) into the defect then it is called lipomyelomeningocele

Caudal Regression

  • Basically fucked up distal spine/sacrum/coccyx with associated spine and cord issues

  • Associated with multiple other congenital fuck shit - especially bladder and renal issues

  • Type 1

    • Conus is blunt and terminates high

    • Severe sacral malformations or absence

    • May have dilattaion of central canal of cord

    • May have cyst on conus

  • Type 2

    • Conus terminates low with tethered and thick filum terminale

    • May have intraspinal lipoma

    • Neuro issues are more severe

    • Honestly looks similar to tethered cord

Fibrolipoma

  • Basically fat and fibrous tissue at distal cord/filum

  • No associated complications, purely incidental

  • Conus terminates at normal level!

Diastematomyelia

  • Basically duplicated cord

Chiari Malformation

Chiari 1

Syrinx

References:

Chiari 2

  • Tectal beaking

Terminal Ventricle

  • Looks like syrinx but only in distal cord

  • Lacks adjacent abnormal cord signal

  • No cord expansion
