Wolman Disease

  • Deposition of fat/foam cells in organs of kids

  • Hepatosplenomegaly + calcified adrenal glands

  • Generalized lymphadenopathy

  • Lethal by 6 months

Adrenal Gland Hemorrhage

  • Most commonly unilateral in setting of trauma

  • Bilateral Adrenal Hemorrhage

    • Less common, think systemic disease like lupus or sepsis

    • Presents with life threatening adrenal insufficiency

Adrenal cortical carcinoma

Adrenal Adenomas

  • Most common masses

  • <10 HU on non-con CT = lipid rich adenoma

  • If not <10 HU may be a lipid poor adenoma or something else

    • CT adrenal protocol to look at washout

    • >60% washout at 15 min = likely adenoma

  • On MR - will have signal drop out on out of phase imaging

  • Management

    • >4 cm = resection

    • 1-4 cm

      • < 20 HU = no f/u required

      • >20 HU = repeat CT in 6-12 months
